Important notice about Televisit (Virtual visit)
We offer televisit (Virtual visit). If you are interested in televisit please notify our office staff. We do not recommend televisit for patients with anorectal diseases, complex problems or cancer because of the necessity of doing more accurate physical exam. Once your televisit is confirmed, we will email you a link to the televisit website. In order to conduct a televisit you will need a smartphone, ipad, tablet, or computer (with webcam) and a high-speed internet or data connection .
Before your first Televisit you need to fill out the following forms:
- For new patients, fill out the “New Televisit Patient Registration Form”. You can download this form or fill out online.
- For existing patients, fill out the “Televisit consent form” . You can download this form or fill out online.
